Accident Claims

Documents required for processing of accident insurance claims:

• Company Claim Request Form – the Insured completes the information on page 1 and the doctor fills in page 2
• X-ray film or copy of X-ray report in the event of a bone fracture or dismemberment
• Other documents, if any, such as a police report

Note: The Insured's signature on the Claim Request Form must be the same as in the insured's policy document.
