  4. Request to Terminate Riders

  5. Request to Reduce Extra Premium

Note: After the contractual change or changes have been recorded, the insurance policy owner will receive the previous Policy Document, with the policy endorsement identifying the details of the change(s) and the new premium amount due attached.
